Vulvar cancer is rare with incidence of 45,240 new cases globally which account for 4% all genitourinary tract neoplasma. It considered as a postmenopausal disease, however age has decreased over the years because high prevalence persistent high-risk human papillomavirus (hrHPV) infection. diagnosis in young women challenging. Prompt and adequate diagnosis, treatment can ensure life quality. A 23 old patient was admitted to Department Operative Gynecology treating malignant vulvar She reffered our clinic electrocauterization after being diagnosed condylomata accuminata. Physical examination revealed vulva atrophy smooth discolored skin patches on both sides. 2 cm exophytic lesion noted ~10 mm from clitoris right labia majora. Multifocal biopsy performed suspect finding proved invasive squamous keratinizing cancer. Positron emission tomography-computed tomography (PET/CT) scan showed individual lymph nodes inguinofemoral region were moderately metabolically active. Wide excision unilateral lymphadenectomy. Macroscopic evaluation entire specimen depicted 2.4 × 2.2 0.5 HPV associated cancer, grade intraepithelial (vHSIL) or usual type neoplasia (uVIN3) lichen sclerosus. Healthcare professionals primary care centers should be adequately trained, aware familiar malignancies younger despite their rarity. Early improve outcomes via reducing morbidity mortality. The individualized surgical preferred strategy patients at present.
